California Consumer Privacy Act FAQ

For all requests, we will need the following information about the consumer:
  • Full Name
  • Date of Birth
  • Phone Number
  • Residential Address

We may collect additional information from you based on our relationship with the consumer in order to properly verify the request by matching the information with data in our systems. This may include email address, last four digits of social security number, insurance policy number, driver’s license or other government-issued identification number or insurance claim number. Only you, or an authorized agent, may make a request related to your personal information. You may also make a request on behalf of your minor child.

For requests that seek the disclosure of specific pieces of information, you may be asked to complete and provide a signed declaration under penalty of perjury that you are the consumer who is the subject of the request. A sample declaration is available for download from the CCPA Web Form.

If you are the parent or guardian of a consumer who is a minor under 13 years of age and would like to request to opt-in for the sale of the minor’s information, you must also provide a consent form to us that is signed under penalty of perjury.
